The University of Utah requires background checks of its students in selected health-related programs prior to matriculation or prior to clinical assignment.  Failure to submit a background check will suspend the matriculation process and/or assignment to a clinical experience.

 

The background check report must be completed through Verified Credentials, Inc.  Please follow the instructions as directed on this Verified Credentials, Inc. website.  You will be required to provide identifying information as well as a payment source for the fee.  You will be directed through the application process and results will be returned to you as well as to your specific health-related program at the University of Utah.  It is important that you submit information in a timely fashion.

 

It is essential that your professional program be able to document this background check for you when requested by hospitals, clinics, and other non-University of Utah training sites.
