The Pennsylvania Department of Public Welfare Child Abuse Clearance must be completed as a separate form.  Please follow the instructions provided on the link and submit your application.  Please note that this process is separate from the Criminal Background Check that will be completed by Verified Credentials and is the student's responsibility.

The Child Abuse Clearance is only valid for one year.  Please submit the Child Abuse Clearance form along with your $10.00 MONEY ORDER(payable to the Department of Welfare) to the Pennsylvania Department of Public Welfare.   The Child Abuse Clearance may take between 3 and 4 weeks to be processed.  The results of the Child Abuse Clearance will be mailed directly to you.  When you receive the results of the Child Abuse Clearance, you must bring the original document to the Temple University School of Pharmacy Director of Student Services and the documentation. Should this document not be provided to the Temple University School of Pharmacy Director of Student Services, you will be removed from your first block.
